We are modifying our copy (Cx18893) of RD Form 3555-21 “Request For Single Family Housing Loan Guarantee” to allow for better form functionality and to more closely follow the GSA/SAM Exclusion instructions on the form which state: “Check Yes if any party is excluded, otherwise check No”.
Modifications to Cx18893 will be as follows:
- If ANY fields for “GSA # Borrower 1-4” (FI36086, FI36087, FI36088, and FI36089) are sent as “1” or “Yes”, the check box for “GSA/SAM Exclusion” on Cx18893 will be checked “Yes”.
- The checkbox for GSA/SAM Exclusion will only check “No” if all of the “GSA # Borrower 1-4” fields were set to “2” or “No”.
- The corresponding “Date GSA/SAM Checked” field(s) will print either a single date (if all checks were ran on the same day) or multiple dates (if any checks were ran on different dates).
- Please note that we will not default to “Yes” or “No” for the GSA/SAM Exclusion fields, the fields must be sent or they will be left blank.
