Document Updates: Various FHA Documents (Cx41 & Cx13513)

Pursuant to our monthly audit of Federal agency forms, we will be updating our copies of the Model Notice for Informed Consumer Choice Disclosure (Cx41) and form HUD-92900-LT (Cx13513) to match the latest versions of these forms (which can be found at: https://www.hud.gov/program_offices/administration/hudclips/forms/ and https://www.hud.gov/program_offices/housing/sfh/model_documents). Substantial changes to these forms include the following:

• • Cx41: Changing the date in the first paragraph to “January, 2021”.
  • Cx13513: Adding “No” checkboxes to the Borrower and Co-Borrower identification areas of the “Underwriting Information” section, as well as revising the form date (to “02/2020”) and expiration date (to “12/31/2023”).
