Pursuant to our monthly audit of federal agency forms, we are updating the following USDA documents to match the latest version of the published model forms:

- - Replacing The Language “Borrower Name” With “Primary Borrower Name”

- Formatted To Double Spacing to Match Model Form on USDA Address.

- An Addition Section Was Added Above Lender Certification Section.
- To Include :
- “TO BE COMPLETED WHEN THE ELECTRONIC LENDER LOAN CLOSING IS OFFICIALLY UNAVAILABLE”
- Primary Borrower Field and Bower ID Field And 4 Co -Borrower Field Along With 4 Co-Borrower ID Fields.
- The Loan Note Guarantee Request Fields Along with Header #3 Have Been Deleted from The Document.
- “LENDER’s” On The “AUTHORIZED REPRESENTATIVE SIGNATURE” Line Has Been Corrected To “LENDER’S”
**Loan Guaranteed Rural Housing Lender Record Change – RD 3555-11 (Cx18894)**
- - Updated Margins to match Model Form
- Removed “Borrower’s Social Security Number from Section “D”
- Moved “As the lender selling or transferring the servicing of the above loan(s), We certify that the information in this submission is true and correct, and that all loans sold were sold to an RHS approved lender” to be seated under Section “D.” As well as associated signature lines.
- Updated Footer to reflect changes made to model form
- Updated formatting under “Instructions for completing Form RD 3355-11”
- Mail, e-mail (priority), or fax all completed forms and any attachments to the address printed on the form. Email was added and indicated as priority.
- Revisions were made to “All blocks, A through D must be completed.” “BLOCK D”
- Email was added to this section
**SFH Loan Guarantee – Form RD 3555-21 (Cx18893)**
- - Edited formatting under “Please issue a Conditional Commitment for Single Family Housing Loan Guarantee in the following case” to meet model guidelines.
- “Application Information” has been replaced with “Primary information in the First Borrower Row
- The 2 Co-Applicant Information sections has been relocated under “Primary Information” Row 1st and 2nd columns.
- “The applicant has does not have a relationship with any current Rural Development Employee.,” “Explain” has been removed from both the Primary and the Co-Borrower.
- Header for consecutive pages has been deleted
- Double space added between “Certifications and Approved Lender Certification”
- “Title / Company” has been moved up to join “Name of Authorized Representative (right aligned) page 2
- Added an additional “Print Co-Applicant’s Name” “Co Applicant’s Signature” and “Date” Signature Lines to page.
