Docutech has added the Ohio Dual Capacity Disclosure to its generic document library as Cx27678 based on the [**sample form**](https://dam.assets.ohio.gov/image/upload/com.ohio.gov/FIIN/CF/fiin_DualCapacityDisclosure.pdf) provided by the Ohio Department of Commerce. Cx27678 will print under the following conditions: State Code (2279) = Ohio Loan Purpose (5991) = Purchase Document Package Type (18215) = Initial Disclosure Dual Capacity (Lender) (40140) = Yes The following new fields have been created for use with Cx27678: Dual Capacity Real Estate Broker Name (188758) Dual Capacity Real Estate Broker License ID (188759) Dual Capacity Real Estate Broker Full Address (188760) Dual Capacity Real Estate Agent/Salesperson Name (188761) Dual Capacity Real Estate Agent/Salesperson License ID (188762) These changes are available for testing on Stage & Stage 2 servers and will take effect on August 7, 2024.
