As part of our monthly audit of federal agency forms, we have made updates to the following document as reflected by the most recent information published by the VA:
– The Respondent Burden in the top right margin has been changed from 10 minutes to 30 minutes.
– The expiration date has been updated to November 30, 2027.
– The Respondent Burden notice language has been revised to align with recent updates from the VA.
– The revision date has been updated from December 2021 to November 2024.
– The superseded date has been changed from July to December.
