Document Updates: Louisiana Signature/Name Affidavit (Cx865) and Louisiana Signature/Name Affidavit (NPE) (Cx7965)

The State of Louisiana requires that the notary block include both the parish in which the notary is qualified and the notary’s identification number. It has come to our attention that the Cx865 Signature/Name Affidavit and Cx7965 Signature/Name Affidavit (NPE) was missing this information when the Security Instrument Notary State (FI 1820) was set to Louisiana. While notaries could manually add these details, for ease of use and consistency across documents, we have added a line for the notary ID and a line to designate the parish where the notary is qualified to both documents.

These changes are available for testing on Stage & Stage 2 servers and will take effect on November 14, 2025. If you have any questions or concerns about these changes, please contact Client Support at 1.800.497.3584.
