Document Updates: Update of Social Security Consent for Release of Information (Cx16668)

Pursuant to our monthly audit of federal agency forms, we have updated Cx16668 Social Security Consent for Release of Information to match the newest model form published in April of 2026 by The Social Security Administration.

The changes are minor and consist of updates to the expiration date, formatting, and updating citations on the form to match current relevant citations.
