Document Updates: Cx3970 – Subterranean Termite Soil Treatment Builder’s Certificate (HUD-NPMA-99A) and Cx3969 – New Construction Subterranean Termite Soil Treatment Record (HUD-NPMA-99B)

After our monthly review we have updated both Cx3970 Subterranean Termite Soil Treatment Builder’s Certificate (HUD-NPMA-99A) and Cx3969 New Construction Subterranean Termite Soil Treatment Record (HUD-NPMA-99B).  Both forms have updated the public reporting burden statement at the top of the page and updated the warning at the bottom of the page.  Cx3670 added a builder signature line with a certification with the signature.  Cx3670 also adjusted the language to the builder installed Subterranean Termite Prevention certification.  Cx3969 added a certification associated with the authorized signature section.
