Document Updates: 4506 Request for Copy of Tax Form (Cx23)

Pursuant to our monthly audit of federal agency forms, we have updated (Cx23) – 4506 Request for Copy of Tax Form.

Included in the (Cx23) updates are:

  • In the top left-hand corner of page 1, the version is now September 2024 instead of November 2021.
  • In Section 8, the fee is now $30/$30.00 instead of $43/$43.00.
  • In the signatory attestation, the phrase “See instructions” is now in bold.
  • In the bottom right-hand corner of page 1, the version is now 9-2024 instead of 11-2021.
  • In the top left-hand corner of page 2, the version is now 9-2024 instead of 11-2021.
  • The states for the Form 1040 series Alabama, Arizona, Arkansas, Florida, Georgia, Louisiana, Mississippi, New Mexico, North Carolina, Oklahoma, South Carolina, Tennessee, Texas, a foreign country, American Samoa, Puerto Rico, Guam, the Commonwealth of the Northern Mariana Islands, and the U.S. Virgin Islands, or A.P.O. or F.P.O. address.
  • The states for the Form 1040 series Connecticut, Delaware, District of Columbia, Illinois, Indiana, Iowa, Kentucky, Maine, Maryland, Massachusetts, Minnesota, Missouri, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island, Vermont, Virginia, West Virginia, and Wisconsin.
  • The states for the Form 1040 series Alaska, California, Colorado, Hawaii, Idaho, Kansas, Michigan, Montana, Nebraska, Nevada, North Dakota, Ohio, Oregon, South Dakota, Utah, Washington, and Wyoming.

These changes are available for testing on Stage & Stage 2 servers and will be in effect on December 4, 2024. If you have any questions or concerns about these changes, please contact Client Support at 1.800.497.3584.
