Document Update: VA Compliance Inspection Report (Cx5402)

Pursuant to our monthly review of government agency and government-sponsored enterprise forms, we have identified an updated version of VA Form 26-1839, Compliance Inspection Report (Cx5402).

The Department of Veterans Affairs (VA) has updated the form with a revision date of July 2026 and an OMB expiration date of July 31, 2029. The VA has also updated their logo on the form. Accordingly, we will be updating Cx5402 – VA Form 26-1839 to match the current VA model form.

These changes are available for testing on Stage & Stage 2 servers and will take effect on September 17, 2026. If you have any questions or concerns about these changes, please contact Client Support at 1.800.497.3584.
