Nebraska Revised Statute 76-1008 allows any person to request notice of default or notice of sale under any trust deed. The request must have the name and address of the person requesting the copies and shall identify the trust deed by stating the original parties, date of filing for record and the book and page or computer system reference. The statute then provides a statutory model form for the request of notice, as shown below:

Request is hereby made that a copy of any notice of default and a copy of notice of sale under the trust deed filed for record ............, 20...., and recorded in book .........., page ........, (or computer system reference ........) Records of ............. County, Nebraska, executed by .......... as trustor, in which ............ is named as beneficiary and ............ as trustee, be mailed to .......(insert name)....... at ..........(insert address).......... .

Signature ..........................

We previously relied on the signature lines for the security instrument. However, to be closer to the model form we are adding a signature line directly beneath the request for notice and prior to the signature lines for the security instrument. We currently have set the borrower to be the person signing this request.
