When a creditor desires to verify the social security number (SSN) of a loan applicant, he may do so by submitting to the Social Security Administration an executed copy of the “Social Security Administration Authorization for the Social Security Administration (SSA) to Release Social Security Number (SSN) Verification” form (SSA-89), published by the SSA on their website (http://www.socialsecurity.gov/cbsv/).
We provide several versions of the SSA-89 form, based upon investor requirements, the most generic of which is Cx5579. We will be making minor edits to these forms in order to match the latest version of the SSA-89 form, which is promulgated on page 53 of the SSA’s “CBSV User Guide,” which can be downloaded through the link set forth above. This version of the form was published in 2008, while the direct downloadable version of the form from the SSA is from 2007.
These changes include the removal of our standard loan information table from the top right-hand section of the document; the removal of the separate date line at the beginning of the document; and the inclusion of the following text in the “I am conducting the following business transaction” line:
“Seeking a mortgage from the Company (Loan #[dynamic loan number])”
Customized text for clients, for this line, will continue to print, unless requested otherwise.
