When a creditor desires to verify the social security number (SSN) of a loan applicant, he may do so by submitting to the Social Security Administration an executed copy of the “Social Security Administration Authorization for the Social Security Administration (SSA) to Release Social Security Number (SSN) Verification” form (SSA-89), published by the SSA on their website (http://www.socialsecurity.gov/cbsv/).
We provide several versions of the SSA-89 form, based upon investor requirements, the most popular of which is Cx5579. We will be making several edits to these forms in order to match the latest (2013) version of the SSA-89 form, which can be found at http://www.socialsecurity.gov/cbsv/#sb=6.
In addition to some minor alterations and formatting changes, the new version will include the addition of a new section “Reason(s) for using CBSV,” with 6 checkbox options: Mortgage Service, Background Check, Credit Check, Banking Service, License Requirement, and Other. Some language on the form has been revised in order to clarify the relationship status of a person completing SSA-89 on behalf of a minor or legally incompetent adult. A new Privacy Act Statement is also incorporated above the Paperwork Reduction Act Statement:
“SSA is authorized to collect the information on this form under Sections 205 and 1106 of the Social
Security Act and the Privacy Act of 1974 (5 U.S.C. § 552a). We need this information to provide the verification of your name and SSN to the Company and/or the Company’s Agent named on this form.
Giving us this information is voluntary. However, we cannot honor your request to release this information without your consent. SSA may also use the information we collect on this form for such purposes authorized by law, including to ensure the Company and/or Company’s Agent’s appropriate use of the SSN verification service.”

The Social Security Administration requires physical signatures on form SSA-89. As part of the update described above, all copies of form SSA-89 in ConformX have been set to only allow for ink signing/wet signatures instead of eSignatures.
