In order to provide verification to a lender of their Social Security number, borrowers are often required to fill out the Social Security Administration’s Form SSA-89, which our Cx5579 is a copy of. We recently updated this document to match the latest version of model form found on the Administration’s website (click here for the announcement).
Some minor deviations were made in Cx5579 from Form SSA-89, in order to fit all the information on the form onto one page, instead of two. Such changes included the deletion of formatting lines and disclosing certain information in two columns, rather than one.
While we believe such minor changes conform with the requirements of 5 CFR § 1320.5(g) concerning modification to forms approved by the Office of Management and Budget (OMB), we have been informed that certain third-party vendors are questioning our Cx5579 because it does not exactly match Form SSA-89. In order to prevent this problem from occurring in the future, we will be modifying Cx5579 to exactly match Form SSA-89. This change will make Cx5579 a two-page document.
In addition to Cx5579, we will be modifying, in the same manner, the following investor and non-standard versions of SSA-89:

- Cx15135
- Cx15856
- Cx16476
- Cx17401
- Cx17442
