RD 3555-21 (“Request for Single Family Housing Loan Guarantee”) is required to be provided by lenders to the RD, in order to obtain a loan guarantee (see RD Handbook HB-1-3555, ch. 10 & 15 for details). We provide RD 3555-21 as Cx18893, a generic document, which prints in both Initial Disclosure and Closing packages. The RD 3555-21 model form was recently updated to remove the initial lines that print at the bottom of each page. In addition, the revision date of the document has changed. We will be updating our Cx18893 to match the updated version of the model form.
