To more closely resemble the model form, we will be making the following minor changes:
- Changing “inquire” to “inquiries” in the first sentence;
- Updating the street address to “2221 Devine Street, 2nd Floor”;
- Deleting the zip code extension on the street address; and
- Updating the Phone number to “803-734-4200 or toll free in SC only 1-800-922-1594”
