Docutech recently learned that a third-party vendor was suspended by the Social Security Administration for 60 days from submitting SSA-89 “Authorization for the Social Security Administration (SSA) To Release Social Security Number (SSN) Verification” forms. This suspension apparently stemmed from the use of “altered” forms. The “alterations” cited consisted of the additions included in the footer of the documents, namely, a form ID number, page numbers, and a bar code. Acting out of an abundance of caution, we have removed all of the footer items from our standard SSA-89 form, Cx5579. To facilitate the organizational and business practices of our clients, we have created an attached coversheet with all of the relevant SSA-89 footer information intact. This coversheet will print with each copy of the SSA-89. Note that this coversheet should not be submitted to the SSA as part of Form SSA-89. This change goes into effect immediately.
We are also in the process of adding the coversheet and removing any potentially problematic “alterations” from all client customized versions of the SSA-89, and will push out these changes as soon as they are completed.
