Kentucky Statute § 304.12-150 stipulates that when insurance is required in connection with a debt or loan on the property, the lender or creditor shall inform the borrower of his or her right of free choice in the selection of the agent and insurer through or by which insurance is to be placed. The Kentucky Department of Insurance uses Form FI-02 (Notice of Free Choice of Agent and Insurer) as a model form to comply with the Kentucky statute. Docutech has updated the Kentucky version of our Cx2778 (Anti-Coercion Insurance Disclosure) to follow this model form.
