Upon review of the MA Important Notice About Your Required Flood Insurance Coverage disclosure (our Cx18930), we decided to update Cx18930 to more clearly resemble the model form which can be found here.
Changes were made per 209 Mass. Code Regs. 57.05 for the following:
1. The heading of the form must be in all capital letters; and
2. In the “Acknowledgment of Receipt,” there must be separate lines for “Borrower’s Signature” and “Date.”
