Pursuant to an audit of the United States Department of Agriculture’s Form RD 3555-11, we have modified our copy of the Lender Record Change form (our Cx18894), to more closely match the latest version from the USDA.
These changes include:
•The paragraph for “Regular Mail” has been removed altogether;
•The address under “Priority Mail” has been changed from the ‘Guaranteed Loan Branch’ to the ‘Housing Services Branch’ in St. Louis, MO;
•The Fax number provided at the top of the form has been changed;
•The Email provided at the top of the form has been changed;
•The “See next page for Instructions” under the title of the document has been changed to “See reverse side for Instructions;”
•The revision date in the upper left corner of the form has changed from ‘Rev.12-14’ to ‘Rev. 02-17;’
•In the lower right hand corner of the fourth box on the form, ‘Zip+4’ has been removed.
