In June of 2016, the Social Security Administration suspended certain third-party vendors from submitting the SSA-89 form because they considered the form to be “altered.” The alterations that were cited consisted of the addition of form ID number(s), page number(s), and a bar code; all located at the bottom of the document. Acting out of an abundance of caution, we removed these items from all of our SSA-89 documents and inserted a cover page containing the necessary information. Following a standard review, we have implemented these same changes to SSA-3288, Consent for Release of Information (our Cx16668). Other minor edits were completed to more closely align with the model form.
