After a recent audit of our IRS and VA forms, the following changes were made to Cx23, Cx3095 and Cx18833 as follows:
Edits to Cx23 – IRS Form 4506 (Request for Copy of Tax Return):
- Page 1/Top Left Header: Revision date changed/updated from “Rev. September 2015” to “Rev. July 2017.” This is a universal change throughout the document; wherever “Rev. 9-2015” appears, it will be replaced with “Rev. 7-2017.”
- Page 1/Section for “Signature of taxpayer(s):” Replacement of old language “For tax returns being sent to a third party, this form must be received within 120 days of the signature date” with new language “This form must be received by IRS within 120 days of the signature date.”
- Page 2/”Specific Instructions” section: Removal of the sentence “If you completed line 5 requesting the return be sent to a third party” from the paragraph beginning “Signature and date.”
Edits to Cx3095 – IRS Form 4506-T (Request for Transcript of Tax Return):
- Page 1/Top Left Header: Revision date changed/updated from “Rev. September 2015” to “Rev. July 2017.” This is a universal change throughout the document; wherever “Rev. 9-2015” appears, it will be replaced with “Rev. 7-2017.”
- Page 1/Section for “Signature of taxpayer(s):” Removal of the sentence “For transcripts being sent to a third party.” Revision of the sentence “this form must be received within 120 days…” to “This form must be received by IRS within 120 days…”
- Page 2/All IRS RAIVS Team addresses have new contact numbers as follows:
Remove “801-620-6922” and replace with “855-298-1145”
Remove “859-669-3592” and replace with “855-800-8015”
Remove “512-460-2272” and replace with “855-587-9604”
Remove “559-456-7227” and replace with “855-800-8105”
Remove “816-292-6102” and replace with “855-821-0094”
- Page 2/”Signature and date” section: Removal of the sentence “If you completed line 5 requesting the information be sent to a third party.” Sentence that beings “IRS must receive…” now begins as “The IRS must receive…”
- ***Please note that custom copies of the IRS 4506-T Form within our library have also been updated with the above listed changes***
Edits to Cx18833 – VA Form 26-6807 (Department of Veterans Affairs Financial Statement):
- Page 1/Right Top Header: Replacement of “Expiration Date: 6/30/2017” with “Expiration Date: 08/31/2020.”
- Page 1/Box for “File No.:” Added “C-“ to precede file numbers.
- Page 1/Bottom Left Footer: Replacement of “JUN 2014” with “AUG 2017.” This is a universal change throughout the document wherever “JUN 2014” appears.
- Page 1/Bottom Middle Footer: Replacement of “OCT 2004” with “JUN 2014.”
