Cx23 is a duplicate of IRS Form 4506 (available directly at: https://www.irs.gov/pub/irs-pdf/f4506.pdf). The newly posted March 2019 version of IRS Form 4506 contains the following changes:
- All references to the September 2018 version now refer to the March 2019 version.
- The “Signature by a representative” paragraph under the “Specific Instructions” section now references Form 2848, line 5a rather than Form 2848, line 5.
- Under the “Chart for individual returns (Form 1040 series)” section, the mail stop for the Kansas City, MO address has been updated to 6705 S-2 from 6705 P-6.
Cx3095 is a duplicate of IRS Form 4506-T (available directly at: https://www.irs.gov/pub/irs-pdf/f4506t.pdf). The newly posted March 2019 version of IRS Form 4506-T contains the following changes:
- All references to the September 2018 version now refer to the March 2019 version.
- The “Caution” paragraph directly beneath line 5b and the “Purpose of form” paragraph under the “General Instructions” section now reference line 5a rather than line 5.
- Under the “Chart for individual transcripts (Form 1040 series and Form W-2 and Form 1099)” section, the mail stop for the Kansas City, MO address has been updated to 6705 S-2 from 6705 P-6.
- Under the “Chart for all other transcripts” section, the Cincinnati, OH address and telephone number have been changed to a Kansas City, MO address and telephone number, and the states have been updated in both sections for the Ogden, UT and Kansas City, MO addresses.
