Document Updates: Various HUD/VA Documents

Pursuant to our periodic audits of our copies of Federal Agency forms, we will be making the following changes (in addition to minor, punctuation changes):

• 203(k) – Draw Request (Cx3399; form HUD 9746-A)
  • Updating the OMB Approval Number expiration date; and
  • Updating references of “Handbook 4204.4” to “Handbook 4000.1”

• 203(k) – Borrower’s Acknowledgment (Cx3406; form HUD-92700-A)
  • Adding the OMB Approval Number and expiration date to the top of the form;
  • Updating both the date in the footer (from “8/95” to “4/2020”) and the reference of “Handbook 4240.4” to “Handbook 4000.1”;
  • Deleting the last bullet point in the first column on page 1 (which references form HUD-314); and
  • Conforming the signature lines with the current format used on form HUD-92700-A.

• HUD/VA Addendum to Uniform Residential Loan Application (Cx20336; form HUD-92900-A and VA Form 26-1802a)
  • The following changes will only be made to the VA-version of the form:
    • Updating the OMB Approval Number expiration date;
    • Updating the VA footer date (from “02/2017” to “03/2020”); and
    • Adding the pronoun “I” to the beginning of Part V, Subsection 25(3)(a) (“I have elected to complete the transaction [etc.]”).
