Document Updates: Several USDA Forms (Cx3818, Cx18891, Cx18892, & Cx18893)

As previously announced, the U.S. Department of Agriculture had posted on their website draft revisions of their Forms RD 3555-17, 3555-18, and 3555-21. USDA Procedure Notice 548 (March 1, 2021) announced that these revisions had been officially made, with the latest versions of the forms available on the USDA’s website.

Due to these changes, we will by modifying our copies of these forms to match the latest versions. Details of these changes are as follows:

• • Loan Note Guarantee – Form CD 3555-17 (Cx18891)
    • • The RHS Case Number area, which appears in the information table on the first page, has been removed. This was done “to protect personal identifiable information by removing the social security number from the form” (USDA PN 548);
      • The revision date has been updated from “04-20” to “01-21”;
      • The email address in Section 8 has been changed from “RD.DCFO.GLB@stl.usda.gov” to “RD.NFAOC.HSB@usda.gov”; and
      • Minor changes have been made to the second-to-last paragraph before the “Conditions of the Guarantee” section. 

• • Conditional Commitment for SFH Loan Guarantee – RD 3555-18 (Cx18892)
    • • The “Borrower SSN” and “(USDA 9-digit ID, not SSN)” areas in the information table on the first page have been removed, “to protect personal identifiable information by removing the social security number from the form.” () The “Borrower ID:” caption within the same table has been revised to “RHS Borrower ID:”;
      • The revision date has been updated from “01-20” to “01-21”; and
      • The “Borrower’s Social Security Number” area in the “Borrower Information” section on the second page has been deleted. 

• • Lender Certification For SFH Guaranteed Loan (Cx3818)
    • • Cx3818 is comprised of only the last two pages of Form RD 3555-18. The only changes made have been to the revision date and to the removal of the “Borrower’s Social Security Number” area, as described above for Cx18892. 

• • Request for SFH Loan Guarantee – Form RD 3555-21 (Cx18893)
    • • The revision date has been changed from “01-19” to “03-21”;
      • The captions “Contact:”, “Phone Number”, and “Approved Lender E-mail:” in the first information table on page one have been changed to “Contact for this File:”, “Contact Phone Number:”, and “Contact E-mail:” (respectively);
      • The second (applicant) information table has been revised to list one applicant and up to three co-applicants (rather than just one co-applicant). The “Social Security Number” and the Veteran status indicators in this table have been removed. The “GSA/SAM Exclusion” portion of this table has been reformatted by placing the checkboxes and date of the GSA/SAM check on a separate line;
      • The property address and the instructions for additional applicants to be documented on a separate form, which both appear after both information tables, have been deleted;
      • The order of checkboxes in Line 1 have been switched and some of the language has been slightly modified;
      • The last, parenthetical sentence in Line 3 has been deleted;
      • Line 5 has been deleted and Line 6 has been renumbered accordingly;
      • The phrase “Only applicable to non-GUS submissions.” has been added to Line 5 (formerly Line 6);
      • The headers on pages subsequent to the first will include the names of the applicant and up to three co-applicants;
      • Additional signature lines and dates have been added, accommodating the applicant and up to three co-applicants; and
      • The “Worksheet for Documenting Eligible Household and Repayment Income”, which comprised three pages of the Form, have been deleted. Per USDA PN 548, this was done to “protect personal identifiable information by removing the social security number from the form.”

Please note that Form RD 3555-21 has been revised to accommodate more than just two applicants. Before this change, Cx18893 was configured to print one copy per every pair of applicants (e.g., one copy for Borrowers 1 and 2, a second for Borrowers 3 and 4, etc.). It will now be configured to print a copy per every four applicants (e.g., one for Borrowers 1 through 4, a second for Borrowers 5 through 8, etc.).
