Pursuant to our monthly audit of VA forms, we will be making the following changes to Cx16049:

    • Changing the OMB Control Number expiration date from “05/31/2021” to “03/31/2024”;
    • Changing the phrase “Your obligation to respond is required” within the first paragraph of Section II to: “Your response is required”;
    • Changing the date within the footer from “MAY 2018” to “MAR 2021”; and
    • Changing the reference “SUPERSEDES VA FORM 26-6381, MAR. 2015” to “SUPERSEDES VA FORM 26-6381, MAY 2018,”.

These changes will take effect on March 31, 2021. Questions or concerns about these changes should be directed to Client Support at 1.800.497.3584.
