Pursuant to our monthly audit of HUD documents, we will be making the following changes to the following documents, to make them conform more with HUD’s official forms:

- Warranty of Completion of Construction – HUD-92544 (Cx47). We will be changing the OMB Approval Number expiration date from “12/31/2023” to “01/31/2024”.
- Builders Certification – HUD-92541 (Cx3971). We will be changing the OMB Approval Number expiration date from “07/31/2022” to “01/31/2024”.
- Conditional Commitment Direct Endorsement (Cx4566 & Cx4733). We will be making the following changes:
- Changing the OMB Approval Number expiration date from “(exp. 12/31/2020)” to “(exp. 01/31/2024)”;
- Replacing the phrase “or the appraisal update” from Clause 3 of the “General Commitment Conditions” section with the following: “(plus 30 day extension when applicable) or 240 days from effective date of the initial appraisal when an appraisal update is completed.”;
- Adding the following as Clause 6 of the “General Commitment Conditions” section: “Certification: The information provided on this form and in any accompanying documentation is certified to be true and accurate.”;
- Updating the FHA Handbook reference in the footer from Handbook 4150.1 to Handbook 4000.1;
- Removing the sentence “This information is required to obtain benefits” from the first paragraph of the “Public reporting burden” section;
- Replacing the second-to-last paragraph of the “Public reporting burden” section with the following: “The information requested is required to obtain a benefit. Responses to the collection of information are required to obtain mortgage insurance. Information contained in this collection will be used only for the purpose of determining the eligibility of a property for mortgage insurance. While no assurances of confidentiality are pledged, HUD generally discloses this data only in response to a Freedom of Information request.”;
- Replacing the last paragraph of the “Public reporting burden” section with the following: “The Conditional Commitment / Direct Endorsement Statement of Appraised Value (Form HUD 92800.5B) sets forth the terms upon which the commitment/direct endorsement statement of appraised value is made and the specific conditions that must be met before HUD can endorse a Firm Commitment for Mortgage Insurance.”;
- Changing the reference to form NPMA-33 in Clause K of the “Specific Commitment Conditions” section to form NCPA-1;
- Changing the reference of “Fee Inspector” to “inspector” in Clause B of the “Specific Commitment Conditions” section;
- Reserving Clause T of the “Specific Commitment Conditions” section; and
- Other non-substantive punctuational, grammatical, and formatting changes.
- Conditional Commitment Direct Endorsement – Homebuyer’s Copy (Cx4734). We will be making the following changes:
- Changing the OMB Approval Number expiration date from “(exp. 05/31/2017)” to “(exp. 01/31/2024)”;
- Adding a new checkbox and the following section to the bottom of page 1 of the form: “Certification: The information provided on this form and in any accompanying documentation is verified to be true and accurate.”;
- Changing Handbook references within the footer from Handbook 4150.1 to Handbook 4000.1;
- Adding the following to the end of Clause L of the “Specific Commitment Conditions” section: “If the mortgage on the property is to be insured under Section 221(d)(2), a code compliance inspection is required.”;
- Replacing the reference of “Fee Inspector” to “inspector” in Clause B of the “Specific Commitment Conditions” section;
- Replacing the reference to form HUD-NPMA-99-A in Clause H of the “Specific Commitment Conditions” section to that of Form HUD-92052;
- Reserving Clause T of the “Specific Commitment Conditions” section;
- Replacing the reference to form NPMA-33 in Clause K of the “Specific Commitment Conditions” section to form NCPA-1; and
- Other non-substantive punctuational and grammatical changes.
Please note that, because our Fulfillment Center services can only print on one side of a sheet of paper, Cx4566 and Cx4734 will continue to deviate slightly from form HUD-92800.5B by making references to either pages 1 or 2 of the form, rather than “on the front/back of this form”.
