Pursuant to our monthly audit of federal agency forms, we are updating our USDA Loan Origination Checklist Attachment 15-A (CX 19212), so that it matches the latest version of the form promulgated in RD HB-1-3555 Ch. 15. This document is available upon request. The changes to this doc are as follows:

  • Replacing the language “The Appraisal report must be uploaded separately,” with “When submitting documents in GUS, the appraisal report must be uploaded separately.
  • The language in the footer was updated from (03-01-21) PN 548 to (05-09-22) SPECIAL PN on all pages.
  • Replacing the language “Documentation of annual income calculations” with “Documentation of annual income calculations” found under the heading Underwriting Analysis: FNMA 1008/FHLMC 1077, or similar form.

Effective Date
