Document Updates: Update to USDA-RD Origination Stacking Order Checklist (Cx19212)

Pursuant to our monthly audit of federal agency forms, we will be updating Cx19212, USDA-RD Origination Stacking Order Checklist, to match the updated form on the USDA-RD website.  The changes include:

• • Updating the Lender Instructions section to match the wording on the new model form
  • Removing the “Evidence of qualified alien” section on page 1
  • Changing the revised date from 05/09/22 SPECIAL PN to 1/9/23 PN 573
  • Moving the header to the left side of the page for page 2
  • Updating the income verification documentation section to remove the option Streamlined: Non self-employed and replace it with Alternative Documentation: Non self-employed
  • Adding the words “and adjusted annual” to documentation required under the “Underwriting Analysis: FNMA 1008/FHLMC 1007, or similar form” section
