Document Updates: Updates to SSA-89 (Cx5579)

Pursuant to our monthly audit of Federal Agency forms we have updated our Cx5579 to match the updated Social Security Administration form SSA-89.  The following changes have been made:

-In the “Privacy Act Statement Collection and Use of Personal Information” Section

• • An added provision that explains sharing of information that is permitted including but not limited to federal agencies to assist in administering SSA programs and employees to perform their duties
  • Added a provision that makes reference to the federal register for the list of routine uses
  • The website at the end of the paragraph was updated to be www.saa.gov/privacy

-In the “Paperwork Reduction Act Statement” section

• • The estimated time to complete the form statement was adjusted from 3 to 20.
  • The estimated time sentence was change from “complete the form” to “read the instructions, gather the facts, and answer the questions.”
  • The send comment section was changed to only allow the address to be used.
