Document Updates: Updates to Request Pertaining to Military Records (Cx17072) & Statement of Purchaser or Owner Assuming Seller’s Loan (Cx21074)

Pursuant to our monthly audit of federal agency forms we have updated VA form 66-6382, Statement of Purchaser or Owner Assuming Seller’s Loan (Cx21074) and Request Pertaining to Military Records (Cx17072) to match the updated model forms from the VA.

The following changes have been made:

Statement of Purchaser or Owner Assuming Seller’s Loan, Cx21074

• • Gray borders have been added
  • The expiration date has been updated
  • (MM/DD/YYYY) has been added wherever a date is asked for to show the format the date should be written in
  • The dates in the footer has been updated
  • The page number has been moved to the right side of the page
  • The format of the section headers on page two have been changed to match the model form

Request Pertaining to Military Records, Cx17072

• • The instructions have been updated to match the new instructions on the model form
  • The website listed on the document has been updated
  • The revision date has been updated
  • The contact information for both the Air Force Personnel Center and Air Reserve Personal Center have been updated
