Document Updates: 4506 Copy of Tax Form (Cx23) & Equal Opportunity Agreement – Form RD 400-1 (Cx7744)

Pursuant to our monthly audit of federal agency forms, we have updated Cx23 “4506 Copy of Tax Form” and Cx7744 “Equal Opportunity Agreement – Form RD 400-1”.

Included in the Cx23 updates are:

• • In the top lefthand corner of page 1, the version is now January 2024 instead of November 2021.
  • In Section 8, the fee is now $30/$30.00 instead of $43/$43.00.
  • In the signatory attestation, the phrase “See instructions” is now bolded.
  • In the bottom righthand corner of page 1, the version is now 1-2024 instead of 11-2021.
  • In the top lefthand corner of page 2, the version is now 1-2024 instead of 11-2021.
  • The states for the Form 1040 series Austin, TX mailing address are now: Alabama, Arkansas, Florida, Georgia, Louisiana, Mississippi, North Carolina, Oklahoma, South Carolina, Tennessee, Texas, a foreign country, American Samoa, Puerto Rico, Guam, the Commonwealth of the Northern Mariana Islands, the U.S. Virgin Islands, or A.P.O. or F.P.O. address.
  • The states for the Form 1040 series Kansas City, MO mailing address are now: Delaware, Illinois, Indiana, Iowa, Kentucky, Maine, Massachusetts, Minnesota, Missouri, New Hampshire, New Jersey, New York, Vermont, Virginia, Wisconsin.

Included in the Cx7744 updates are:

• • In the top righthand corner of page 1, the new OMB No. 0575-0189 is beneath the old OMB No. 0575-0201.
  • In the top righthand corner of page 1, the Expiration Date is now 3/31/2026 instead of 07/31/2025.
  • The boxed and italicized paragraph at the bottom of page 1 now appears as:

“According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a valid OMB control number. The valid OMB control number for this information collection is 0575-0201 and 0575-0189, which expire 3/31/2026. The time required to complete this information collection is estimated to average 10 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection information. All responses to this collection of information are voluntary. Any questions on this burden can be sent to ICRMTRequests@usda.gov.”

• • The page numbering at the bottom of all pages has been removed.
  • The form information “RD 400-1 (Rev. 5-00)” only appears in the bottom righthand corner of page 1, and it no longer appears on any other pages.
