Pursuant to our monthly audit of federal agency forms, we have updated Cx16049 “Application for Assumption Approval” – VA Form 26-6381.
Included in the Cx16049 updates are:

- In the top righthand corner of the page, the Expiration Date is now “01/31/2027” instead of “03/31/2024”.
- In the paragraph above Section I, the words “INSTRUCTIONS:” and “NOTE:” are now bolded.
- The borders around Sections I & II now appear in the color gray.
- In Section II, the phrases “PRIVACY ACT NOTICE:” and “RESPONDENT BURDEN:” are now bolded.
- In the last sentence in Section II, the final phrase now reads “suggestions for this form.” instead of “suggestions about this form.”
- In box 2A, it now reads “NAME OF SELLER (First – Middle – Last) (Type or Print)” instead of “FIRST-MIDDLE-LAST NAME OF SELLER (Type or Print)”.
- In box 3A, it now reads “TELEPHONE NUMBER” instead of “HOME TELEPHONE NO.”
- In box 3B, it now reads “EMAIL ADDRESS” instead of “WORK TELEPHONE NO. (Include Area Code)”.
- In box 5, the alignment is now left justified instead of centered.
- In box 5A, it now reads “NAME OF ASSUMER OF LOAN (First – Middle – Last) (Type or Print)” instead of “FIRST-MIDDLE-LAST NAME OF ASSUMER OF LOAN (Type or Print)”
- In box 6A, all of the phrases after the three checkboxes are now in lowercase rather than capitalized.
- In box 7A, the phrase “(Check one)” now reads “(Check one if applicable)” and the checkboxes for “MONEY ORDER” and “OTHER (Specify)” now appear beneath “PAYMENT ENCLOSED FOR CREDIT REPORT AND/OR PROCESSING CHARGE”.
- In box 8A, the word “LENDER” now appears as “SERVICER”.
- In box 10, bolding and italicization have been added to the phrase “(NOTE: Lien is a legal claim against a property that must be paid off when the property is sold. Assessment is a tax or levy imposed against specific parcels of real estate that can benefit from improvements such as a street or sewer.)”
- In box 11, the question now reads “HAVE VA FORMS 26-6382 AND 26-6807 BEEN PROVIDED TO YOUR PURCHASER FOR COMPLETION?” instead of “HAVE YOU DELIVERED VA FORMS 26-6382 AND 26-6807 TO YOUR PURCHASER FOR COMPLETION AND FORWARDING TO VA?”
- In the paragraph beneath box 11, the word “IMPORTANT:” is now bolded.
- In box 12A, the phrase “(Sign in ink)” has been removed.
- In box 12B, it now reads “DATE (MM/DD/YYYY)” instead of “DATE”.
- At the bottom of the page, the form version date “MAR 2021” is now “JAN 2024”, the superseded form date “MAY 2018” is now “MAR 2021”, and the page number alignment is now right justified instead of centered.
