Pursuant to our monthly audit of federal agency forms, we have updated Cx19212 “USDA/RD Origination Stacking Order Checklist – Attachment 15-A.” and Cx18894 “Guaranteed Rural Housing Lender Record Change – RD 3555-11”.
Included in the Cx19212 updates are:
- Relocated from the top righthand corner of all pages to the top lefthand corner of all pages is the information “HB 1-3555, Attachment 15-A, Page (1) of 3.”
- The Rural Development logo which used to be at the right of the USDA logo has been removed.
- Under the “Loan Origination Checklist – Guaranteed Underwriting System (GUS) – Purchase and Non-Streamlined and Streamlined Refinance Transactions – Underwriting Recommendation: ACCEPT” table, in the second row, the title “FEMA Form 086-0-32, Standard Flood Determination Form” has been replaced with the title “FEMA Form FF-206-FY-21-116, Standard Flood Hazard Determination Form.”
- In the bottom lefthand corner of all pages, “Revised (06-28-23) PN 585” has been replaced with “Revised (04-01-24) PN 609.”
- Under the “Loan Origination Checklist – Manual Underwriting, GUS Refer/Refer with Caution Underwriting Recommendation, or GUS Accept with Full Documentation Message on GUS Underwriting Findings and Analysis Report” table, in the second row, beneath the title “Underwriting Analysis: FNMA 1008/FHLMC 1077, or similar form” has been removed the instruction “Executed by the approved lender’s underwriter.”
- Under the “Loan Origination Checklist – Manual Underwriting, GUS Refer/Refer with Caution Underwriting Recommendation, or GUS Accept with Full Documentation Message on GUS Underwriting Findings and Analysis Report” table, in the tenth row, the title “FEMA Form 086-0-32, Standard Flood Determination Form” has been replaced with the title “FEMA Form FF-206-FY-21-116, Standard Flood Hazard Determination Form.”
- Under the “Loan Origination Checklist – Manual Underwriting, GUS Refer/Refer with Caution Underwriting Recommendation, or GUS Accept with Full Documentation Message on GUS Underwriting Findings and Analysis Report” table, the eleventh row of “Evidence of qualified alien” has been entirely removed.
- Under the “Loan Origination Checklist – Streamlined Assist Refinance Manual Underwrite Review” table, the eighth row of “Evidence of qualified alien status is required when adding a new borrower who is not a U.S. Citizen” has been entirely removed.
Included in the Cx18894 updates are:
- In the top lefthand corner of page 1, “(Rev. 08-22)” has been replaced with “(Rev. 04-24).”
- In the top righthand corner of page 1, “Exp. Date 12/31/2024” has been added beneath the OMB No.
- Under the title “Lender Record Change,” the priority mail and fax number information has been entirely removed.
- Under the title “Lender Record Change,” “EMAIL: RD.NFAOC.HSB@usda.gov” has been replaced with “Email the completed Form to: RD.SO.HSB@usda.gov.”
- In the box at the bottom of page 1, the previous text has been entirely replaced with:

- Under “Instructions for Completing Form RD 3555-11, “Guaranteed Rural Housing Lender Record Change,” the instruction “Mail, e-mail (priority), or fax all completed forms and any attachments to the address printed on the form.” has been replaced with “E-mail all completed forms and any attachments to RD.SO.HSB@usda.gov.”
- Under “Block D,” the bolded sentence, “If more than one loan is being transferred, and Blocks A and C are identical, the lender may attach a list of all borrowers affected by the transfer that includes all the requested information and deliver by mail, e-mail, or fax.” is now unbolded and reads, “If more than one loan is being transferred, and Blocks A and C are identical, the lender may attach a list of all borrowers affected by the transfer that includes all the requested information and send to RD.SO.HSB@usda.gov.”
