A modification of the FNMA Form 1003 Uniform Residential Loan Application (cx4193) occurred May 27, 2011 to print the Loan Origination Company Name, NMLS Identifier, and Address in the Loan Origination Company boxes near the end of the fourth page if they had a value, and to continue to print the Lender Name, License Number, and Address only if any of the Loan Origination Company information was empty.
Even though FNMA specifically requires the NMLS Number for the Loan Origination Company, some state regulators are still looking for the state license number. Therefore, starting July 30, 2011, both the Lender/Broker License Number and the Loan Origination Company NMLS ID will print in the “Loan Origination Company Identifier” box near the end of page four if they have values.
