The IRS updated Form 4506 “Request for Copy of Tax Return” (cx23) and Form 4506-T “Request for Transcript of Tax Return” (cx3095). The new 2012 versions include various textual changes, including a cautionary statement to only sign if all applicable lines have been completed.
In addition, revised IRS Form 4506-T includes a checkbox to indicate that a requested transcript involved identity theft on the federal tax return.
The updated documents will be available February 4, 2012.
