Cx28407 “IL Disclosure of Financial Interest” is based on the model Form DS-1 from the Illinois Department of Financial and Professional Regulation. Immediately preceding the “Acknowledgment” section, it states:

The undersigned does hereby certify that the above disclosure was made to the above named party(ies) on

Signature of Producer: _________________________ Date: ____________________

The newly created field “Producer of Title Business - Individual Agent Signer Role(FI 193995) is now available to designate the eSign role of the individual eSigning in the “Signature of Producer” section. The eSign role options under FI 193995 are: 

  • Loan Officer
  • Underwriter
  • Corporate Officer
  • Lender Representative
  • Loan Originator
  • Interviewer
  • Broker
  • Escrow Officer
  • Loan Closer
  • Settlement Agent
  • Closing Agent

Key details:

      • The eSign role will continue to default to “Loan Officer” if FI 193995 is left blank during the eSign process.
      • The field “Producer of Title Business – Individual Agent Name” (FI 193697) will continue to be utilized for the name of the person signing in the “Signature of Producer” field during the eSign process.
      • If the “Signature of Producer” is ink-signed, neither the Individual Agent Signer Role (FI 193995) nor the Individual Agent Name (FI 193697) fields will be used.

This change is available for testing on Stage & Stage 2 servers and will take effect on March 24, 2026. If you have any questions or concerns about these changes, please contact Client Support at 1.800.497.3584.
