Document Updates: USDA/RD Origination Stacking Order Checklist – Attachment 15-A (Cx19212)

Pursuant to our monthly audit of federal agency forms, we have updated Cx19212 “USDA/RD Origination Stacking Order Checklist – Attachment 15-A.

The following changes have been made to Cx19212:

• The Lender Instructions on the top of the form have been updated to match the model form.
• The Loan Origination Checklists have all been updated to include updated form names, more detailed instructions, and to remove any forms no longer needed.
• Minor formatting changes throughout the document have been made.

These changes are available for testing on Stage & Stage 2 servers and will take effect on September 19, 2025. If you have any questions or concerns about these changes, please contact Client Support at 1.800.497.3584.
